Happy November
Can you believe it's already November???! Time sure is going by fast and soon we'll be half way through the school year! So far we've done lots of things at school and we look forward to many more!
In regards to your children's health we have conducted vision screenings and dental varnishing (for elementary schools). We will soon be having hand-washing and hygiene classes to remind our students how they can continue to stay healthy and avoid communicable diseases.
Now I'd like to take the time go over a few important things:
Vision Screening Follow Up
Most vision screenings have been done by now and letters have been sent out to the parents of those students who failed the screening. If your child failed their vision screening a complete eye exam is recommended. If you received a letter please let me know if you have any questions. Letters need to be returned to the school nurse once the student has had an eye exam, and an ophthalmologist or optometrist has completed the form at the bottom. I will be following up throughout the school year. At the end of the school year this information is then reported to the State. Thanks for your cooperation!
Immunizations
If you have not provided the school with an immunization record or exemption form for your student, it is important that you bring one to the school as soon as possible. It is State law that every student registered in school provides proof of up-to-date immunizations or an exemption form in order for students to attend school. We have already started sending out letters to those who are still missing proper documentation regarding this matter. Please refer to the link below to see what immunizations are required for school attendance in the state of Utah. Or if you need an exemption, click on the link for exemptions. If you are obtaining an exemption for your student, after completing the module, there is a certificate that needs to be printed. It is important that you bring this certificate to the school as we have no other way of knowing that you have completed the module.
